Admissions Statement
The Chisago Lakes Achievement Center (CLAC) shall not refuse to admit a person solely on the basis of the type of residential services a person is receiving or solely on the basis of the person’s severity of disability, orthopedic or neurological handicaps, sight or hearing impairments, lack of communication skills, physical disabilities, personal care needs or behavioral challenges. The CLAC will not discriminate and no exclusions will be made on any basis prohibited by law, including race, color, sex, age, religion, national origin, disability, marital status or veteran status.
Admissions Criteria
Have desire to attend and/or the applicant’s Interdisciplinary Team (IDT) concurs that placement at the Chisago Lakes Achievement Center is appropriate.
18 years of age or older
Have one or more documented disabilities.
The Chisago Lakes Achievement Center must have the necessary resources available to meet the assessed health, safety, and service needs of the individual referred.
Must reside within the service area of the CLAC's transportation system, or be able to consistently provide for their own transportation.
Have had a medical evaluation within the past 12 months. (The date of this evaluation must not exceed more than 365 days prior to the applicant's actual starting date.)
Have had a psychological evaluation as determined by the IDT within the past 3 years. A more recent psychological evaluation may be required if the applicant has had a history of mental illness, has undergone a significant change in status, functioning level, medical condition, etc., or has experienced any other condition which may significantly affect the person’s psychological status as determined by the members of the Interdisciplinary Team. The Interdisciplinary Team may also waive the requirement for a psychological exam within the past 3 years provided that the reason for this waiver is documented.
Be eligible for a funding source, or reach an agreement with CLAC for private payment for services rendered.
Chisago County Human Services must concur that placement of any applicant not residing in Chisago County is appropriate for the delivery of services to that individual.
Have a letter from their County Social Worker stating that the training and habilitation services to be provided are not replacing services that are the statutory responsibility of a local educational agency or that are otherwise available from a rehabilitation agency funded under section 110 of the Rehabilitation Act of 1973, United States Code, Title 29, section 730 as amended through October 31, 1988.
